Instructions

1. Write an essay on your family background and future plans. What is leadership? Why do you deserve this award?
2. Send a copy of your unofficial transcript.
3. Send the name and email of one teacher/mentor/manager/coach who will be writing a letter of recommendation for you. Letters should be emailed by the writer to dave@nexlevprep.com. If you cannot find someone to write a letter of recommendation for you, send a short explanation about why.
